Begin with a call or email
The admissions process begins with a confidential conversation. When you contact Foundations Minnesota, a member of our admissions team will take time to understand your needs, explain available services, and help determine the appropriate level of care. From your first call forward, the goal is to provide clear information so you can make a well-informed decision about treatment.
Intake assessment call
To begin admissions, we speak directly with the person seeking treatment. Referrals from loved ones, clinicians, or employers are welcome but the intake assessment must be completed by the client.
During this approximately 20-minute call, we gather information about substance use history, mental health concerns and current needs. All information shared is confidential even if you choose not to move forward with treatment. If you need support from an interventionist, the admissions team can provide referrals to trusted local and national resources.
Verify insurance and payment options
After the intake assessment, our team will review insurance benefits and discuss available payment options. Insurance verification is often completed within a few hours. In some cases, insurers may request an additional assessment, which our team will help coordinate. Details about coverage, accepted insurance plans, and self-pay options are available.
Foundations Minnesota has two outpatient locations
- Eden Prairie. Outpatient services for adults 18+ addressing substance use and co-occurring mental health concerns
- Minneapolis. Outpatient services designed to support the LGBTQ+ community facing substance use, mental health and sexual health concerns, using the PRIDE Program curriculum.
