Outpatient Addiction Treatment Levels
Foundations Minnesota offers a full continuum of outpatient addiction treatment services. Individuals may begin at different levels of care depending on their clinical needs and can transition between programs as recovery progresses.
Partial Hospitalization Program (PHP)
PHP is the highest level of outpatient care offered at Foundations Minnesota.
PHP provides structured daily programming while allowing individuals to return home or to supportive housing in the evenings.
PHP may be appropriate for individuals who:
- Are stepping down from inpatient or residential treatment
- Need more support than traditional outpatient care provides
- Are managing both substance use and mental health concerns
- Benefit from a more structured treatment environment early in recovery
PHP services may include:
- Group therapy
- Individual therapy
- Medication management support
- Psychoeducational programming
- Family support and education
- Relapse prevention planning
- Ongoing clinical assessment and care coordination
Intensive Outpatient Program (IOP)
IOP provides structured therapy and recovery support several days each week while allowing individuals to continue managing work, school, family, and other daily responsibilities.
- IOP services may include:
- Group therapy
- Individual therapy
- Cognitive behavioral therapy (CBT)
- Motivational interviewing
- Psychoeducation and recovery skill development
- Medication management support
- Family involvement and support programming
IOP is available at both our Minneapolis and Eden Prairie locations. The Minneapolis campus also offers LGBTQ+ focused outpatient programming through the PRIDE Program.
Virtual Outpatient
Virtual IOP services are available throughout Minnesota through secure telehealth programming.
Virtual treatment may be helpful for individuals who:
- Live outside the Twin Cities
- Have transportation barriers
- Need greater scheduling flexibility
- Prefer receiving treatment remotely
Individuals participating virtually continue working directly with their Foundations Minnesota clinical team throughout treatment.

Many individuals experiencing addiction are also managing anxiety, depression, trauma, PTSD, bipolar disorder, or other mental health concerns. Foundations Minnesota provides integrated treatment designed to address both substance use and mental health together through coordinated care and individualized treatment planning.
Family Program
Addiction impacts the entire family system, not only the individual experiencing substance use. Our family programming helps loved ones better understand addiction, recovery, communication, boundaries, and ongoing support needs.
Family services may include:
- Psychoeducation about addiction and recovery
- Communication and boundary-setting support
- Family therapy sessions
- Relapse education and support planning
- Resources for loved ones and caregivers
LGBTQ+ Affirming Care
Both Foundations Minnesota locations provide LGBTQ+ affirming care. Our Minneapolis location also offers LGBTQ+-exclusive programming through the PRIDE Program, one of the longest-running LGBTQ+ focused addiction treatment programs in the country.

You May Be a Good Fit for Outpatient Treatment If:
- You have a stable or supportive living environment
- You are medically stable and do not require detox services
- You want to maintain work, school, or family responsibilities during treatment
- You are transitioning from inpatient or residential treatment
- You are motivated to participate in therapy and recovery programming
- You have transportation access or can participate virtually
Things to Consider:
- Active detox needs may require a higher level of care first
- Unstable or unsafe living environments can make outpatient treatment more difficult
- Severe or long-term addiction may require residential stabilization
- Acute psychiatric crises may need additional support before outpatient treatment begins
